Health Disparities Collaborative (HDC)

National Program for Treatment of Chronic Diseases

The Health Disparities Collaborative (HDC) is a national program for the treatment of chronic diseases at a primary care level in which providers of community health centers follow a Care Model to deliver health care. The Care Model is unique in that it involves patients’ participation in self-management of the chronic disease to achieve self-chosen goals directed towards improvement of their chronic conditions. Results of participation in the Health Disparities Collaborative are intended to reduce the cost of health care and to improve the lives of patients treated under the program.

Family Health Network has been engaged in the program since 2002 in the treatment of depression at our DeRuyter Health Center. In April 2006, FHN “spread” its participation in the HD Collaborative to the Moravia Health Center using the Care Model in the treatment of diabetes. More recently, the DeRuyter Health Center has added diabetes to its HDC Care Model while also continuing its treatment of depression.
