Quality Health Care for Your Whole Family

The mission of Family Health Network of Central New York, Inc. (FHN) is to provide quality health care primarily to the residents of Cortland and adjacent counties, with a particular emphasis on underserved areas. FHN's team of healthcare professionals includes Board Certified or Board Eligible Doctors, Nurse Practitioners, Physician Assistants, Registered Nurses, Licensed Practical Nurses, Psychiatrists, Certified Social Workers, Family Counselors, Dentists, and Dental Hygienists.

Sliding Fee

We offer a Sliding Fee program to qualified families. Download a Sliding Fee Application.

Assistance

We can help you enroll in other programs for low-income patients, such as Child Health Plus, Family Health Plus, and Medicaid for Adults. Call our Facilitated Enroller at (607) 662-0381.
