Payment Information

Payment Policies

Family Health Network of Central New York, Inc. (FHN) welcomes all patients regardless of their insurance status or ability to pay. We participate with most major insurers, including Medicare and Medicaid. Please bring your insurance card with you for every visit. We also offer a Sliding Fee Program for the uninsured (Call (607) 753-3797 for more information).

You are responsible for paying for non-covered services, co-payments or unmet deductibles in full at the time of your visit. Payment in full is also expected at the time of your visit if FHN does not participate directly with your insurance plan. At the time of your visit, you may pay by cash, personal check, or credit card—Master Card, Visa, or Discover.

Payment Assistance Programs

FHN wants you to get the health care you and your family need. We offer assistance through our Sliding Fee Program that matches the cost of your care with your ability to pay based on your income and household size. For more information call (607) 753-3797. FHN can also help you enroll in other programs for low-income patients that are listed below. To register or to find out more about the programs listed below, please call our Facilitated Enroller at (607) 753-3798.

Child Health Plus

A low or no cost health insurance program that covers most medical expenses for children under age nineteen (19) or pregnant women who live in New York State. You may qualify for Child Health Plus A (Children's Medicaid program) or Child Health Plus B.

Family Health Plus

Available to adults between the ages of 19 and 64 who do not have health insurance, but have incomes too high to qualify for Medicaid.

Medicaid for Adults

A health insurance program that offers adults complete coverage for all their health care needs. No monthly payment is required.
