FHN has been awarded BOTH the Gold+ and Silver Awards from the American Heart Association (AHA) for the Target BP program in 2024 based on 2023 quality data!

The data submitted reflected impressive HbA1c control in patients, and appropriate statin therapy use throughout our at-risk patient population.
