Community Health Quality Recognition (CHQR) badges recognize Health Center Program awardees, like Family Health Network (FHN), that have made notable achievements in the areas of access, quality, health equity, health information technology, social risk factors screening, and COVID-19 public health emergency response using data that is required to be reported annually to Health Resources & Services Administration (HRSA).

FHN are 2023 Awardees of the following badges:

  1. The Bronze Health Center Quality Leader badge puts FHN in the top 21-30 % of all federally qualified health centers in the nation.
  2. The Advancing Health Information Technology (HIT) for Quality badge indicates that FHN met all HIT criteria including offering telehealth services, engaging patients through health technology systems, collecting patient social risk factor data, and more.
  3. The Patient Centered Medical Home (PCMH) badge suggests that FHN provides comprehensive primary care services via multidisciplinary providers and staff to improve health equity and patient outcomes.
  4. The National Quality Leader for Maternal & Child Health badge was awarded to only 12 health centers out of 1,370 nationwide. FHN is the only federally qualified health center that received this award in New York State.  This recognizes exceptional care in areas such as prenatal care, birth weight, immunizations, and more.
